Choose a Unit with the lowest current HP.

Does this second effect's "choose 1 enemy Unit with the lowest HP" mean I choose one Unit with 3 HP when there are two Units with 3 HP and one Unit with 5 HP on the enemy's field?

Yes. When there are multiple Units with the lowest HP, choose one of them.
